Form4 Terms & Conditions – Platform Rules and Usage Guidelines
This document sets out the terms and conditions governing the use of the form4 platform. By registering or using any of its services, you agree to comply with all provisions contained herein. Please read carefully before proceeding with registration.
Effective date: 1 January 2026. These terms may be updated from time to time.
1. Acceptance of Terms & User Eligibility
By accessing or using the form4 platform, you confirm that you have read, understood, and agreed to be fully bound by these terms and conditions. If you do not agree with any part of these terms, you are required to refrain from using this platform.
To register and use form4's services, users must:
- Be at least 18 years of age at the time of registration.
- Is legally competent to enter into a binding agreement in the jurisdiction where you reside.
- Is not on any prohibited or sanctions list issued by any relevant authority.
- Register one account only — creating multiple accounts per user is strictly prohibited.
Form4 reserves the right to request identity verification at any time to confirm user eligibility. Failure to provide the required verification documents may result in account restriction or closure.

2. Account Creation, Security & User Responsibilities
Account Creation & Maintenance
During registration, you are required to provide accurate, complete, and up-to-date information. Submitting false or misleading information may result in account suspension without prior notice.
You are responsible for keeping your account password confidential. form4 will not be held liable for any losses resulting from unauthorised access caused by your failure to safeguard your login credentials.
Key User Responsibilities
- Do not share your account credentials with anyone.
- Do not use the platform for fraudulent activities, money laundering, or any other unlawful purpose.
- Do not attempt to hack, disrupt, or compromise the security of the platform.
- Do not use automation software, bots, or third-party scripts to interact with the platform.
- Notify the support team immediately if you suspect unauthorised access to your account.
3. Platform Usage Rules & Prohibited Activities
The form4 platform is intended solely for lawful entertainment purposes. Users are prohibited from doing the following while using this platform:
- Using software designed to exploit system errors or vulnerabilities.
- Manipulating games through technical means or by colluding with other users.
- Attempting to circumvent the platform's security systems or verification measures.
- Using VPN or proxy connections to conceal your actual location for deceptive purposes.
- Creating multiple accounts to unfairly exploit promotions or bonuses.
- Threatening, harassing, or behaving in an offensive manner towards staff or other users.
- Posting content that is unlawful, obscene, or infringes upon the rights of third parties.
- Engaging in any form of money laundering through this platform.
Violation of any of the above rules may result in immediate account suspension or closure, forfeiture of account balance, and legal action where applicable.
4. Payments, Deposits, Withdrawals & MYR Transactions
All financial transactions on the form4 platform are processed in Malaysian Ringgit (RM). Users are responsible for ensuring that deposited funds are their own and have been obtained lawfully.
Transaction Terms
- Deposit: All deposits are final once confirmed. Successful deposits cannot be reversed unless a technical error is acknowledged by the platform.
- Withdrawal: Withdrawal requests are subject to completed identity verification (KYC). form4 reserves the right to request supporting documents before approving any withdrawal.
- Bonus Rollover Requirements: Any bonus or promotional credit applied to your account is subject to the rollover requirements stated in the respective promotion's terms. If those requirements are not met, the bonus will not be eligible for withdrawal.
- Processing Time: Withdrawals are typically processed within the timeframe stated on the platform's help page. Processing times may vary depending on the chosen payment method and verification process.
- Transaction Limits: Minimum and maximum limits for deposits and withdrawals are subject to the selected payment method and the user's account tier.
- Currency: form4 is not liable for any losses arising from currency conversion carried out by your bank or any third-party payment provider.
5. Liability Limits & Account Suspension Policy
Platform Liability Limits
Form4 is not liable for any indirect losses, consequential damages, loss of profits, or other damages arising from the use of or inability to use this platform. The platform's maximum liability, under any circumstances, is limited to the amount of active deposits held in the user's account at the time of the incident.
The platform does not guarantee uninterrupted service availability. Scheduled maintenance, technical disruptions, or unforeseen circumstances may result in temporary service downtime.
Account Suspension & Closure
Form4 reserves the right to suspend or close any user's account without prior notice in the following circumstances:
- The user has breached any of the terms set out in this document.
- There is suspected fraudulent activity, money laundering, or misconduct.
- The user fails to provide the requested identity verification information or documents within the stipulated timeframe.
- Account has been inactive for a continuous period exceeding 12 months.
If an account is closed due to a terms violation, any remaining balance may be forfeited subject to the outcome of an investigation by the platform's security team.
Intellectual Property
All content on the form4 platform — including brand names, logos, interface designs, text, graphics, and software — is the exclusive property of the platform. Users may not copy, redistribute, modify, or use any of these elements without explicit written consent from form4 management.
Third-Party Links & Services
This platform may utilise third-party services such as payment providers and game suppliers. form4 is not responsible for the privacy policies, terms of use, or conduct of any such third parties. Use of third-party services is entirely at the user's own risk.
6. Changes to Terms & Governing Law
Right to Amend Terms
Form4 reserves the right to amend, add to, or remove any part of these terms and conditions at any time without prior notice. Changes take effect immediately upon being published on this page. You are encouraged to review this page periodically to stay informed of the latest version of our terms.
Continued use of the platform after any changes are published will be considered acceptance of the updated terms. If you do not agree with the changes, you should stop using the platform and close your account.
Governing Law
These terms and conditions are governed by and interpreted in accordance with applicable law. Any dispute arising in connection with these terms or the use of the platform shall be resolved through negotiation first before any legal action is pursued.
Should any provision of these terms be found invalid or unenforceable by a court of competent jurisdiction, that provision will be reinterpreted or removed without affecting the validity of the remaining provisions.
